Existing clients, please use this link to access your secure client portal where you can schedule appointments, view and pay bills, and contact your provider through secure messaging.

New Clients

How to Schedule:

1. You may click the schedule appointment button above to be directed to a link where you can request an appointment time with your preferred provider or, Complete Request for Services Form or call us at 515-446-7284 to schedule an appointment and start the onboarding process
2. You will receive an email from a no-reply@valant.com to set up your client portal if you have completed the online request form.
3. Once your portal is set up, practice paperwork with policies and consent to provide care will be sent to your client portal for completion and you can request an appointment with your preferred provider indicated on the form.
4. You may complete our practice paperwork online or you may arrive to your first appointment early to complete in person.
